This form is to be used by candidates who are required to recredential before being eligible to sit Fellowship Examinations. For more information or to confirm if this form applies to you, please contact the Examinations Team.

Fellowship candidates who are required to recredential must submit this form by 31 October the year prior to the Examination year they intend to sit.
